HAUNTED HOUSE PARTICIPANT WAIVER AND RELEASE OF LIABILITY
I hereby acknowledge that I am voluntarily participating in the haunted house experience provided by Dead Fellows Haunted Brewhouse, a service of Strange Fellows Brewing (hereinafter referred to as the "Business"). I understand that the haunted house experience may involve physical and psychological challenges, loud noises, strobe lights, fog, special effects, and other potentially frightening and disorienting elements.
In consideration for being permitted to enter and participate in the haunted house experience, I, on behalf of myself and any personal representatives, heirs, and next of kin, hereby acknowledge and agree as follows:
1. Assumption of Risk: I understand and acknowledge that participating in the haunted house experience may involve risks and hazards, including, but not limited to, slipping, tripping, falling, collision with objects or other participants, and exposure to potentially frightening and intense situations. I voluntarily assume all such risks.
2. Release of Liability: I hereby release, discharge, and hold harmless Dead Fellows Haunted Brewhouse, Strange Fellows Brewing, its owners, employees, volunteers, and agents (collectively referred to as the "Released Parties") from any and all liability, claims, demands, actions, or causes of action, whether in law or in equity, arising out of or related to my participation in the haunted house experience, including but not limited to personal injury, disability, death, property damage, or any other loss.
3. Covenant Not to Sue: I agree not to initiate or pursue any legal action or lawsuit against any of the Released Parties based on any claim arising out of or related to my participation in the haunted house experience.
4. Medical Authorization: In the event of an injury or medical emergency, I authorize the Business and its representatives to seek and obtain medical treatment on my behalf.
5. Photographic and Video Release: I consent to the use of any photographs, videos, or recordings of me taken during my participation in the haunted house experience for promotional and marketing purposes by the Business.
I have read and understand this waiver and release of liability, and I voluntarily agree to its terms.
